Organophosphates are one of the most commonly encountered poisoning agents but their combination with pyrethroids is the least studied. The combination of chlorpyrifos 50% and cypermethrin 5% poisoning cases were studied to analyse the demographic profile and clinical features at presentation thereby forming the basis for treatment and the outcome. Material and Methods: It is a hospital based retrospective study of chlorpyrifos-cypermethrin combination poisoning cases which were brought to the Emergency Department of AIIMS, Bhubaneswar. Results: This study analysed 106 cases of chlorpyrifos-cypermethrin poisoning from April 2022–March 2024. Most cases (78.3%) involved combined poisoning, with 84.9% being intentional. Key symptoms included excessive salivation (p = 0.0005), lacrimation (p = 0.0140), respiratory crepitations (p = 0.0026) in mixed poisoning, head reeling in chlorpyrifos cases, and seizures in cypermethrin cases (p < 0.0001). The survival rate was 97.86%, with a 2.84% mortality rate. Conclusion: Chlorpyrifos-Cypermethrin poisoning causes severe cholinergic symptoms, requiring urgent care. Predominantly intentional among young males, it underscores the need for stricter regulations, better healthcare access, and mental health support.
